Staj raporu nasıl yazılır?

Raporun ilk sayfası, pratik çalışmayı yapan öğrencinin adını, soyadını, numarasını, bölümünü, yapılan stajın numarasını içeren kapak sayfası olmalıdır. Rapor Türkçe veya İngilizce olarak yazılabilir. Raporlar ciltli veya dosya içerisinde teslim edilir.

Staj raporu nereden alınır?

Staj için sağlık raporu 'nu yalnızca işyeri hekimleri düzenleyebilir. Aile hekimleri bu belgeyi veremezler.

Staj raporu en az kaç sayfa olmalı?

STAJ RAPORU YAZIM KURALLARI. Staj raporu aşağıda bulunan soruların yanıtlandığı, staj yapılan işletmede öğrencilerden yapılması istenen özel çalışmaların ve çizimlerin bulunduğu, ekler hariç, en fazla 15 sayfalık bir rapordur. Staj raporu A4 boyutundaki beyaz kağıda bilgisayar kullanılarak yazılmalıdır.


Staj raporu sonuç kısmına ne yazılır?

Sonuç bölümü, gerçekleştirilen projeden veya yapılan stajdan elde edilen sonuçların değerlendirildiği bölümdür. Bu bölümün yapısı şöyle olmalıdır: Proje veya staj konusu tanımlanmalı ve kullanılan yöntemler özetlenmelidir. Elde edilen sonuçlar açık ve basit cümlelerle ifade edilmelidir.

Staj raporunda ne yazılır?

Kapak sayfası, stajyer öğrencinin adı, soyadı, sınıfı, numarası, stajın konusu (yazılım/donanım), başlama ve bitiş tarihi (gün/ay/yıl), süresi (x hafta/ x*5 iş günü), staj yapılan yerin adı ve adresi bilgilerini içerir.

Staj raporu ne demek?

Staj raporu, staj kılavuzundaki soruların yanıtlandığı, staj yapılan işletmede öğrenciden yapması istenen özel çalışmaların ve çizimlerin bulunduğu rapordur.

Staj sigortası e devlette görünür mü?

TC kimlik numarası ve e-devlet şifresi ile kimlik doğrulaması yapın. E-devlet şifreniz yok ise PTT şubelerinden alabilirsiniz. Ardından otomatik olarak yönlendirileceğiniz hizmet dökümü sayfasında staj sigortanız ile ilgili bilgileri görebilirsiniz.

Stajyer sağlık raporu zorunlu mu?

Sağlık Kurul Raporunda (Heyet Raporu) mutlaka "Ağır ve Tehlikeli İşlerde Çalışabilir, Sağlık Kurumunda Staj Yapmasında Bir Engel Yoktur, Hastanede Çalışabilir, Hastanede Staj Yapabilir'' vb. ibareler olmalıdır.

Staj yaptığına dair belge nereden alınır?

(SGK İşe Giriş Bildirgesi Hakkında) Staj yapacak öğrencilerimiz; SGK İşe Giriş Bildirgelerini www.turkiye.gov.tr(e-devlet) adresinden staja başladıkları gün temin edebilirler.

Günlük staj raporu nasıl yazılır?

Staj yaptığınız gün ve stajın kaçıncı günü olduğu raporun sağ üst kısmındaki TARİH kısmına yazılmalıdır. Örneğin “24.06.2019-1.gün” olarak yazılmalıdır. 9. Yapılan uygulamalar ekran alıntıları ile ilgili gün ve sayfada yer almalıdır.

Staj rapor hakkı kaç gün?

Yaz stajında devamsızlık hakkı var mı? Raporsuz devamsızlık hakkı yoktur. Raporlu olarak ise en fazla 4 gün devamsızlık yapılabilir. Devamsızlık yapılan günlerin telafisi hemen staj bitiminde, stajın son gününü takiben (ara verilmeden) aynı kurumda yapılır.

Staj defteri kabul edilmezse ne olur?

6) Staj defterini zamanında teslim etmeyen öğrencilerin stajları geçersiz sayılacaktır. 7) Staj çalışması kabul edilmeyenler, bu çalışmayı yeniden yapmak zorundadır. Stajını tamamlamayan öğrencilere, sorumlu oldukları diğer ders ve uygulamaları başarmış olsalar bile, diploma veya mezuniyet belgesi verilmez.

Stajda rapor nereye verilir?

Staj Raporu. Staj sonunda öğrenci, bölümün istediği düzende bir staj raporu hazırlamakla yükümlüdür. Bu rapor stajın bitiminden itibaren en geç iki ay içinde Staj Koordinatörlüğü'ne teslim edilmelidir.

6. Staj raporun son hali ayrıca pdf dosyası şeklinde hazırlanmış halde bir CD içerisinde staj komisyonuna teslim edilmelidir. Raporlar staj komisyonu tarafından iki kez kontrol edilir.
